MEMBERSHIP AGREEMENT

This contact is between A&D Appliance, 284 N 8th St, Prospect Park, NJ 07508 and {CustomerName},{CUSTOMERADDRESS}.

1. Scope of service applies to all household appliances (washer, dryer, oven, refrigerator and etc.)

This provides:

3 free service visits (trip and diagnostic) per year.

10% percent off parts (not allowed on any special orders, oversized items and cosmetic items.

WE ALSO WE PROVIDE PRIORITY SCHEDULING

2. CONTRACT DURATION

This contract shall commence on 01/18/2025 remain in effect till 01/17/2026 unless terminated earlier per terms of contract 

3. Payment Terms

The total fee for this yearly membership is $199.00 payment shall be made as follows:

Payments are due annually upon the year's maturity.

Payments will auto-renew and customer will be responsible to update credit card and billing information to complete transactions annually to maintain membership. 

4 Responsibilities of Contractor;

The CONTRACTOR SHALL

• Provided qualified personnel to preform service
• Maintain records of repairs done
• Respond to service request with 24 hours of working days

5 Responsibilities of the client

The Client shall

• Provide access to the appliance for maintenance and repairs
• Notify the technician of any issue with appliance must be presented on the initial visit cannot be combined at any time or date
• Ensure that the appliances are in suitable condition for service and maintenance

6. Termination:

Either party may terminate contract with 30days notice either written verbally with said company

7. The contractor shall not be liable for any damages arising from the use of appliances, except in cases of gross negligence or willful misconduct

8 This contract shall be governed by the laws of the said state the agreement is signed and activated in.
